Selon une \u00e9tude effectu\u00e9e en 2003 par la U.S. <em>Joint Commission on Accreditation of Healthcare Organizations<\/em>, pr\u00e8s de 70 % de tous les \u00e9v\u00e9nements sentinelles sont caus\u00e9s par une non-communication. (Alvarado, K, et autres, 2006) C\u2019est pendant le transfert de responsabilit\u00e9 (parfois d\u00e9sign\u00e9 par \u00ab transfert de soins \u00bb) qu\u2019il existe le plus grand risque de tort pour le patient ou client.<\/p>\n<p>C\u2019est \u00e9galement pendant ce transfert que les manquements \u00e0 la <strong>confidentialit\u00e9<\/strong> des renseignements personnels des patients ou clients se produisent souvent. Ce n\u2019est pas seulement la question de manquer ou de mal comprendre l\u2019information qui doit \u00eatre transmise, mais aussi la question que parfois certains membres du personnel qui ne devraient pas \u00eatre mis au courant le sont. Il est essentiel, lorsqu\u2019on divulgue des renseignements personnels sur la sant\u00e9, de se rappeler de qui fait partie du <strong>cercle de soins<\/strong> du patient ou client. Cette expression n\u2019est pas d\u00e9finie dans la <em>Loi de 2004 sur la protection des renseignements personnels sur la sant\u00e9<\/em>, mais on admet g\u00e9n\u00e9ralement qu\u2019elle d\u00e9signe l\u2019ensemble des fournisseurs de soins de sant\u00e9 qui offrent des soins et services aux fins de traitement primaire des patients ou clients. Elle d\u00e9signe \u00e9galement les activit\u00e9s connexes comme les travaux de laboratoire et la consultation aupr\u00e8s d\u2019autres fournisseurs de soins de sant\u00e9\u00b2.<\/p>\n<p>Certaines organisations se sont dot\u00e9es de m\u00e9thodes normalis\u00e9es fond\u00e9es sur les preuves afin d\u2019am\u00e9liorer l\u2019efficacit\u00e9 et la coordination des communications dans le transfert de responsabilit\u00e9. Certains services d\u2019h\u00f4pital ont un format genre liste de v\u00e9rification (code, exigences de lutte contre les infections, pr\u00e9occupations quant aux risques) afin de s\u2019assurer de ne rien oublier d\u2019important lors du changement. La porte est ouverte et il y a dans le couloir un va-et-vient du personnel infirmier qui arrive au travail ou qui termine. En outre, la patiente ou cliente est la belle-soeur du technicien en \u00e9chographie qui est dans la salle du personnel au moment o\u00f9 le th\u00e9rapeute respiratoire raconte l\u2019histoire. Elle doit \u00eatre pr\u00e9sente physiquement pour fournir un rapport de vive voix, \u00e0 moins qu\u2019il n&rsquo;existe un autre m\u00e9canisme pour ce faire.
